LESSONS WILL NOT BE GIVEN WITHOUT A SIGNED CONTRACT
New students/parents are required to read the AFM Vocal Studio, Inc. Policies, and a signed contract must be received before, or on the day of the first lesson. 

• Please complete the Policies Contract, and bring it to your first lesson, or fax it to (801) 415-5169
